IgG4-related disease (IgG4-RD) is a systemic fibroinflammatory disorder that may involve cervical lymph nodes, mimicking neoplastic or infectious conditions. We report the case of a 68-yearold man with progressive cervical lymphadenopathy, weight loss and fatigue. Imaging revealed lymph node conglomerates, and pulmonary biopsy showed dense plasmacytic infiltration with IgG4-positive cells, confirming the diagnosis. Glucocorticoid therapy was initiated, resulting in favorable clinical evolution. IgG4-RD should be considered in the differential diagnosis of persistent lymphadenopathy. Accurate diagnosis relies on a comprehensive assessment integrating clinical, serologic and histopathological findings.
